Wigmore Family Health Center
PROVISION OF MEDICAL DOCUMENTS
At the "Wigmore Family Health Center", the following services are provided free of charge, along with the corresponding documentation:
- Organization of medical care and outpatient treatment for individuals of pre-conscription and conscription age.
- Provision of medications for individuals entitled to free or preferential access to medicines.
- Periodic medical examinations of teachers in general (state) schools and caregivers, educators, and nannies in preschool (state) educational institutions (sanitary booklet).
- Assignment of students in state higher education institutions to physical education groups (main, preparatory, special, and therapeutic).
- Administration of preventive vaccinations (both routine and epidemiologically indicated).
- Hospitalization of patients according to medical indications (electronic referral).
- Organization of post-hospital treatment and care.
- Temporary disability evaluation.
- Referral for assessment of an individual’s functional capacity.
- Issuance of medical documents (extracts, certificates, etc.).
The center also provides the following services on a paid basis:
- Pre-employment (upon hiring) and periodic (during employment) medical examinations.
- Issuance of medical certificates for obtaining a firearm permit.
- Issuance of medical certificates regarding the health status of adults traveling abroad.
- Issuance of medical certificates regarding the health status of individuals applying for child adoption or guardianship.
- Issuance of medical certificates regarding the health status of individuals applying for temporary or permanent residence or legal status in the Republic of Armenia.
- Provision of medical certification for individuals holding or applying for a driver’s license.
- Issuance of medical certificates confirming the absence of physical defects or diseases that would prevent appointment as a judge, prosecutor, or state prosecutor employee.
- Issuance of medical certificates confirming the absence of diseases that would prevent the performance of duties in public service positions or pedagogical/administrative activities in educational institutions.
- Medical care and services for foreign citizens registered in primary healthcare institutions.
